Referring Clinic Portal Registration Instructions

Please provide the information requested below. Fields with an asterisk are required. Be sure to indicate if you are requesting Laboratory access, Teaching Hospital access, or both. Because the security of your information is important to us, all requests require individual confirmation. If you do not receive a confirmation and welcome email within one business day, please contact us.

Please note that each clinic will have a single portal account for viewing both hospital medical records, as well as Athens Diagnostic Lab results. If you already have a portal login for DLab results, and would like access to hospital medical records, registration is not required. Please email ugaportal@uga.edu with your request for additional access.

By applying to register for a clinic portal account, you are declaring yourself as an official representative of the clinic for which the request is made. It is unlawful to knowingly provide false, incomplete, or misleading information in order to fraudulently gain access to confidential medical and financial records, such as through this portal.
